But in order for him to plea guilty, he has to prove that he understands that he’s waiving his right to trial by jury. I think it’s for his protection—to make sure he is completely aware of the decisions he’s making.
Yep. And without being forcibly medicated, he wouldn’t. And he would not be allowed to strike a plea, either. He was found incompetent to stand trial previously.

You are correct. and so the ethical conundrum in American crim law. If a person is too deranged to stand trial he/she is treated by doctors and nurses to recover his/her sanity so that he/she can stand before the bar of justice and be convicted.
